Copies of birth and death certificates for those who were born or died within the corporation limits of the City of Hamilton are available through the Health Department. This typically includes only births at the old Mercy Hamilton or Fort Hamilton Hospitals.
Birth and death certificates may be requested by telephone, mail (please include a self-addressed stamped envelope) or in person for a fee of $25.00. Cash, personal check, money orders or Visa and Master Card are acceptable forms of payment.
Please provide the following information when requesting a birth certificate:
- Date of Birth
- Full Name
- Father's Name
- Mother's Name, including maiden name
- Telephone number
Please provide the following information when requesting a death certificate:
- Deceased's Full Name
- Date of Death
- Place of Death (i.e. Hospital, residence)
